Your Care, Our Team

When you choose CHA, you get an entire Primary Care team. It's all about putting you first, at the center of your care. Your team is led by your "primary care provider" or "PCP", usually a doctor or nurse practitioner. It also includes nurses, medical assistants, front desk staff, therapists, pharmacists and others.

Your CHA care team works together to support your needs. This may include seeing you in the office, sending you for tests or referring you to a specialist. They also help you make decisions about the best options for care.
